- Proof of identity (we must see your child’s birth certificate, and with your permission, copy it for our files)
- Proof of physical examination by a physician within 12 months of start date
- Proof of state required immunizations
- Signed parental agreements
- Payment of enrollment fees ($75 annual registration), due each year on anniversary date
